About This Assessment

This assessment gathers teacher observations of the student's behavior in the school setting. Your input is essential because ADHD must be present in multiple settings (both home and school) for a proper diagnosis. Teachers provide unique insight into how students function compared to same-age peers in structured academic and social situations. This form assesses inattention, hyperactivity/impulsivity, oppositional/conduct behavior, and emotional functioning. This assessment takes approximately 10 minutes to complete.
